I'm thinking that every piece of technology in the area of effect that could malfunction would have a chance of malfunctioning. In GURPS, that would be a roll against 18-(TL of the device); a failed roll means consult the appropriate Malfunction table. A catapault (TL2) would fail on 17 or 18, a Colt .45 (TL6) would fail on 13 or higher, a phaser (TL12) would fail on 7 or higher, and so on...
