A 'Gentleman's Gentleman' is exactly what it sounds like.  He is more than a butler, more than a valet.  They are the left hand men, just out of sight until called upon, and always proactively serving their master.  That said, I do believe that Reginald Jeeves may also qualify.  The only difference being that there isn't much anything extraordinary about Bertie Wooster ( at least to my knowledge anyhow).
