One would assume that Section 4 of the Twenty-fifth Amendment applies, although that section has never been used.
(It might even apply if he's simply uttering bald-face lies. Apparently, that almost happened in 1987, when it was claimed that Reagan was "inattentive" and "inept".)
EDIT: Oh, and just in case anybody doesn't think that Papadopoulos is the the big news in this, here's an analysis from the outside looking in.
