Tetsurashian's Full Circle has updated a few times since I last checked it, this being the most recent. It's one of those with a weedy sort of plot growing in the crack, Potterverse based. Harry and Tom have been soul-bound and reincarnated so many times that they've gotten used to and then fond of each other, now on their 89th lives they are once again Harry Potter and Tom Riddle. They've been the enemies of canon, they've been siblings, parent and child, and frequently as time wears on, lovers and spouses as the years and lives and bodies come and go. This time, they're milking the situation for all the amusement value they can wring out of it, though the way major elements of their first lives keep cropping up despite what should have been decisively heading them off is a matter of growing concern.
As long as the gay-cooties don't put you off, it's quite a fun read. Even if they do a little, it's easy enough to apply a Rule-63 perception filter to Harry (And Remus I suppose, though he's barely in it) without changing any actual plot elements.
