In the In Nomine canon, where the Archangels perform Redemptions, the failure (and thus death) rate is approximately 1/3. In OMB, where the big guy personally supervises the job... who knows?
And even when the ex-demon survives, the loss of those Forces makes the new angel noticeably weaker than before he, she, or it Redeemed.
