I think that wishes would still be bound by his normal rules - ie no effect further away than his AoE, conjured items/beings vanish at the end of the song, people or animals who appear because of it are not the actual thing, just acting how the wisher and/or Doug's subconscious expect them to act in that situation. Objects physically transformed from existing matter would be an exception, if only as accurate as Doug and/or the wisher could make them using their own knowledge and any reference material inside AoE, and at GM's discretion this could apply to living beings transformed by the effect also, with intelligent beings making the relevant checks to resist/overcome mental attacks to revert to their normal mental state if "reprogrammed" ala simulacrum-brain once when it happens and again at the end of the song if the transformation is permanent.
