The thing with Momo is that she has spent insane amounts of time with Aizen... which means she has been exposed to his mind hacking far more than anyone else. Basically she got brain damage from extreme over exposure to his abilities. Aizen has been making desisions for her for so long she that when he left she went into withdrawl. With herorin the body stops making endorphins... as its getting more than it needs. So when it stops getting the artificial sorce it goes through a period f having no endorphins produced at all. Which causes miserable pain... because the body in question isn't making any pain killers and is over reacting to anything remotely like pain. Momo has had that issue... only its a matter of thinking and making decisions. So in that crossover Momo may literally not notice it isn't Aizen... if Aizen didn't tell her to specifically notice the hammer isn't Aizen. Depending on when they get switched the hammer could show up as Aizen in everyones minds... so when they raid Soul Society in the early arcs... its entirely possible that Momo ferries the hammer everwhere and everyone actually thinks it is Aizen.
This wouldn't be an illogical crack fic so much as Aizen's plans carrying on and he just not planning on randomly being abducted by extrademennsional forces.
One thing you have to remember with Bleach is that its a Nippon Ichi style Netherworld equivalent. The entire point of Soul Socity and the surrounding territory is to process souls that don't want to move on to reincarnation. So the area sucks so bad you give up all hope of existing an let your sense of self die... or even will yourself to die. This fails to work for those with spirit power... thus the Reapers which are used to whittle away at those that won't stop existing on their own. The purpose of Soul Society is to ger Reapers killed. I'm pretty sure the gates of hell could have dragged that serial killer that was using the boy possessing the bird as bait... the hollows and reapers exist as a purgatory processing center... the 11th makes perfect sense this way... the captainy passes to whoever manages to kill off the last stupidly power dead guy leading the group.
This also makes the illogical, ever shifting back story and seemingly retconning the timeline every arc or so... not to mention Yamato's dickery. Talk up their duty and immortality... then let them get themselves killed off. Ichigo warped this dynamic, but triggered so many wars and high level encounters that he is making up for it for now.

I don't know how old Schlock Mercenary is, but I'm fairly sure Captain Simian and the Space Monkeys came first with their red-shirted Holoboons. At least, it was off the air before I was on the net mid-to-late 90s.
